DC Healthy Families Program
Keep your health benefits! Renew your family’s DC Healthy Families Program benefits each year with these simple steps:
Step 1: Check your mail
You’ll receive a renewal form from the Department of Human Services (DHS).
- Look for your form up to 90 days before your benefits will end.
- Moved? Make sure DHS has your current address. Call 202-727-5355 (TTY 711).
Step 2: Complete the renewal form
Complete the renewal form when you receive it.
- Fill out all the information on each page.
- Be sure to sign the signature page.
Step 3: Return the renewal form on time
Return the renewal form by the due date.
